Section 8 Mainstream Housing Opportunities for Persons with Disabilities Program
Through this program, New Avenues continues to address the housing needs of individuals with disabilities. The HUD-funded program, was established to provide rental vouchers to enable persons with disabilities to rent affordable private housing of their choice.
New Avenues’ Mainstream program currently assists up to 125 individuals or families with disabilities in Cuyahoga, Lake, and Ashtabula counties.
In addition to the rental assistance provided through the vouchers, we provide housing search assistance, tenant/landlord mediation, and technical assistance to landlords for making accessibility modifications to units.

PLEASE NOTE: New Avenues has not received additional vouchers. We are simply adding applicants to our waiting list. We will only be housing approximately 5-10 individuals and families this year.
To be considered for our program, download the pre-application, complete it and submit it following the instructions. Applications must be completed, in ink, by the Head of Household. The Head of Household must be an adult (18 years or older) with a disability and the legal capacity to enter into a lease. If a guardian or other adult completes the application for the Head of Household, the Head of Household must sign the application on page 6 verifying the accuracy of the information.
Pre-Application
Mailing InstructionsNew Avenues will rank all applications based on preferences and date of postmark. An application must show proof
of mailing consisting of one of the following:(1) A legibly dated U.S. Postal Service Postmark.(2) A legible mail receipt with the
date of mailing stamped by the U.S. Postal Service, indicating what was mailed.(3) Postage to return the completed application is
1 first class postage stamp.New Avenues does not accept either of the following as proof of mailing:(1) A private metered postmark,
or(2) A mail receipt that is not dated by the U.S. Postal Service.
Waiting list order is determined by date of postmark on the application received by New Avenues and number of accumulated local preferences based on disability and income. One application per applicant. Duplicates will be discarded; the application with the earliest date will be kept by New Avenues. Applications postmarked the same date with the same local preferences will be ranked based on a lottery process and placed on the wait list accordingly. Applicants will be notified of their rank on the wait list by New Avenues.

Broadmoor Therapeutic Summer Camp
New Avenues operates the Broadmoor Therapeutic Summer Camp in Lake County for over
110 children and youth. The program is an integrated program for children ages 3-21. It serves children with special needs as
well as typically developing students. The five week program provides individualized instruction with a 4:1 child to teacher ratio.
The children are divided by age into preschool, primary, intermediate and advanced groups.
Swim, Gym & More Program
New Avenues operates the Swim, Gym & More Program in Lake County for 38 children and youth.
The program is an integrated program for children ages 3-22. It serves children with special needs as well as typically developing
students. The program operates during the school year on alternating Saturdays. The Saturday program curriculum includes swimming,
arts & crafts, snacks, gym, outdoor play, fine motor, and music activities.
Through the Adult Day and Senior Programs, adults with mental retardation and/or developmental
disabilities are afforded the opportunity to participate in individual and group programming designed to meet the needs, honor personal
choice and achieve goals of each participant. They include an emphasis in the areas of life skills, sensory integration, and work
activities and are complemented by the provision of leisure/recreational activities, motor development, aquatics, art and music therapies.

Edgerton, in University Heights, is home to eight adults with mental retardation and/or developmental disabilities.
Facility-Based Residential Program
Through traditional residential services, New Avenues’ 88 residents live in seven group homes and two ICFMRs owned and operated by New Avenues. In addition to having their basic needs of food, clothing and shelter met, each of the residents served by New Avenues has been evaluated to determine what services they need, as well as desire. An Individual Habilitation Plan (IHP) is then tailored to that individual so that he or she will be able to obtain personal goals and develop the skills necessary for more independent living. New Avenues has a professional staff and consultants who provide individualized services for each client.
Supported Living Program
Through the Supported Living and Community Based Waiver programs, New Avenues provides in-home services for children and adults with mental retardation and/or developmental disabilities that differs from traditional residential services. An individual’s needs and preferences determine the supported living services required to assist the individual in successfully living in the community.
In Individual Service Plan (ISP) is created to identify what services are needed. The ISP is developed by a team consisting of the individual, the individual’s case manager, a residential services representative, the individual’s guardian and any other person(s) that the individual wants involved. The ISP assesses needs in the following eight areas: choices and options, hygiene and appearance, interpersonal relationships, community activities, income, housing, health, and safety.
Respite Program
New Avenues' Transitional Respite Program provides support and relief to informal caregivers, including families
and friends, coping with the stress of caring for a dependent person. Through this program, we provide transitional housing and support
services for up to four individuals for a period of two weeks to six months.
The summer curriculum is divided into five weekly themes and the children participate in art, music, physical education, swimming,
computers and weekly special events. In addition to the primary curriculum, students can receive assistance from occupational therapy,
physical therapy, music therapy and speech/language therapy.
Therapeutic Services
New Avenues offers a variety of therapeutic services to clients. These services include music, language,
art, occupational, physical and recreational therapies which are available to all clients.

New Avenues’
Nursing Department provides a wide variety of health care services to our clients including general and skilled nursing care, health
care education, monitoring of infection control program, and implementation of Physician’s Orders.
