New Avenues' Supported Living Program is a residential service option for individuals with developmental disabilities. Supported Living is different than traditional residential services. An individual’s needs and preferences determine the Supported Living services required to assist the individual in successfully living in the community.
In 1992, we were approved to provide services in Lake, Cuyahoga, Geauga, Summit, Medina, Trumball, Ashtabula and Lorain Counties. We first provided services in Lake County in 1992, then added services in Cuyahoga County during 1993. In 1997, the agency expanded into Ashtabula County. Currently, we serve more than eighty individuals throughout Lake, Cuyahoga and Ashtabula counties.
Eligibility
Children or adults with mental retardation and/or developmental disabilities may be considered for Supported Living services in Northeast Ohio if:
The individual meets criteria established for individual options waiver and/or supported living funding
The County Board funder approves
the individual for Supported Living funding
The individual wants to participate in the Supported Living program
Referral Process
Most referrals for Supported Living services are received by New Avenues from Northeast Ohio County Boards of Mental Retardation. However, arrangements can also be made directly with New Avenues for private referrals.
Determination of Supports
An individual service plan (ISP) is developed by a team of people consisting of the individual, the county board support representative, a residential provider representative, the individual’s guardian / family member(s), and any other person(s) that the individual wants involved.
The ISP identifies the level of supports that the individual will require to be assisted with to live successfully in their home in the community. Our services range from providing several hours per week within that individual’s home to providing services and supervision 24 hours a day.
